The parent/legal guardian named in this registration form approves of his/her child’s or children’s participation in South East Toronto Flag Football and/or their own participation and agrees that the player(s) and all family members, at all times, will abide by the rules, regulations and the codes of conduct for the South East Toronto Flag Football and the City of Toronto parks and rec. Failure to comply with the rules, regulations and the codes of conduct may result in withdrawal of rights as a member of this organization. The above named parent/legal guardian also agrees on his/her own behalf and on behalf of his/her child or children not to hold the South East Toronto Flag Football, its officials, executive, directors, agents, volunteers or referees responsible for, or to make any claims, demands or commence any actions or proceedings for damages, loss, injury or costs experienced by the player(s) or the parent in consequence of his/her/their participation in or presence at any programs, games or activities of South East Toronto Flag Football.

By completing this form you are consenting to the collection, use and disclosure of the personal information contained herein for purposes related to the organization and administration of South East Toronto Flag Football, including its on­line registration process.

Photo Release Consent required: On occasion the SETFF administrators and/or related members (players, parents/legal guardians, officials, or hired photogra­phers) may photograph and/or record your child’s or children’s image on still or motion picture film, and/or on audio tape, and use it for promotional purposes. By agreeing to this release you waive any claim to remuneration for use of these materials for this purpose. Please note that we will inform our constituents if/when we actively pursue this option to promote the league and its program(s).
